Please read, complete & submit the following questionnaire to determine your eligibility for any coaching service with KarmicPharm. Read through the questions carefully and be sure to review the coaching options under the “services’ section of this site.

Hypnosis/Hypnotherapy Intake Form
Fill out this form to purchase Hypnosis/Hypnotherapy from KarmicPharm
Purchasing means that you Understand and accept all of the following terms and conditions for Clinical Hypnosis and Hypnotherapy from KarmicPharm.
Complete & submit this form for approval to KarmicPharm. Once your application for Hypnosis/Hypnotherapy has been approved
you will receive an email from KarmicPharm to schedule your pre session Zoom call.
– You must be 18 years old or older to purchase Hypnosis/Hypnotherapy sessions
– Hypnosis/Hypnotherapy is effective for:
*breaking habits (smoking, etc.)
*phobia management (fear of flying etc.)
*anxiety relief (calm intrusive thoughts etc.)
*stress reduction (manage work/life balance etc.)
*confidence construct (eliminate self- -doubt etc.)
– You are not a candidate for Hypnosis/Hypnotherapy if you are or have been under psychiatric care
– You are not a candidate for Hypnosis/Hypnotherapy if you are prescribed anti- depressants
– Hypnosis/Hypnotherapy will only work if you are committed to the change you’re seeking and you’re an open & receptive participant during your session
– It is possible that some Hypnosis/Hypnotherapy participants might require more than one session to achieve their desired outcome
– Once your Hypnosis/Hypnotherapy application is submitted & approved, you’ll receive an email to schedule a pre session call via Zoom
***This call is required to schedule your Hypnosis/Hypnotherapy session date
***Upon completion of the Zoom call, a $50.00 deposit is required to book your Hypnosis/Hypnotherapy session. This deposit is applied to the overall amount of your session with KarmicPharm. This deposit is non-refundable.
Final payment for your session is required 3 days in advance of your scheduled Hypnosis/Hypnotherapy session date. Refunds are not available.
